Pediatric Lymphedema Management Certification Course Agreement

By registering for this course, you agree to the following terms:




CLT Requirement

Participants are required to be CLTs to enroll in this course. Please email a copy of your CLT certificate to [email protected] after completing the registration process. If you do not complete this at least 4 weeks prior to the course start date, your registration fee & your spot in the course may be forfeited.



Confirmation

Participants will receive a confirmation email to the provided email address with the exact start & end times for their course, as well as specific location information.




Class Times & Attendance

Classes will run from 8am-6pm local time. Participants are expected to attend the entirety of the course to receive their certificate of completion.



Dallas Course Information

The Dallas location course is held at The Health Collective (10670 N Central Expy Suite 110, Dallas, TX, 75231).

We recommend staying at Home2 Suites by Hilton Dallas North Park (2 minute walk from The Health Collective).



Phoenix Course Information

More details will be released closer to the course dates.




Certificate of Completion

Participants must pass all course requirements to receive their certificate of completion. Course requirements include attendance, written exam, & skills check-offs.




Refund Policy

$150 of registration is non-refundable. Four weeks prior to the course start date, the complete registration fee is non-refundable. Refunds will be issued 4-6 weeks from the date of request.




Withdrawal

Withdrawal from the course may result in a loss of registration fees. If the participant is unable to complete the course due to personal emergency or medical reasons, the student will have 12 months from the original course date to return to the in-person course & complete it.


Class Cancellation

If the course is cancelled due to insufficient attendee registration, participants will receive a refund or they may transfer their registration to a different course of their choosing. If a course is cancelled due to instructor personal emergency or medical reasons, a new course will be scheduled & participant registration fees will be applied to this new course.




Materials Sharing Policy

The participant agrees to be the sole individual accessing the course materials. Sharing of the course materials with other individuals or organizations is strictly prohibited.






CEU Information
